If you would like your Soul Essence Portrait, you can get started by completing the following Soul Visions Portrait Questionnaire. The questionnaire can also be downloaded (questionnaire.pdf - PDF format) and printed out to be submitted in written form.

Please note: Lyn Mason requires a color photo for all potential clients (good shot of the face) along with the completed questionnaire to complete an accurate Soul Essence Portrait. Upon completion of the questionaire you can email a copy of your photo to info@soulvisionart.com
